The ATSB has released its preliminary investigation report into the Collision between passenger train and platform at Cleveland, Qld, 31 January 2013 On 31 January 2013, a Queensland Rail passenger train failed to stop at the Cleveland station platform and collided with the end-of-line buffer stop, the platform and the station building at a speed of about 31 km/h. A company has been fined $60,000 (plus $7673 in costs) over an incident in which a female worker was injured when a lifting device fell onto her from the rear of a truck. National Fleet Administrative Services Pty Ltd pleaded guilty to failing to provide and maintain a safe work environment for a person not being an employee of National Fleet Administrative Services and, by that failure, causing serious harm to the person, and was fined in the Perth Magistrates Court on February 14. Double propeller overspeed involving a Bombardier DHC-8, VH-SBV near Weipa Qld, 6 December 2011

A Bombardier DHC-8-315 aircraft, registered VH-SBV and operated by QantasLink, was on a scheduled flight from Cairns to Weipa, Queensland. The aircraft was on descent with the power levers in the flight idle position and the first officer’s hand was on the power levers. When the aircraft encountered turbulence, the first officer inadvertently lifted one or both of the flight idle gate release triggers and moved the power levers below the flight idle gate. Engineering company fined $10,000 over lack of licence

A Henderson engineering company has been fined $10,000 for allowing workers to do high risk work when they did not hold a licence for that class of work. KEP Management Services Pty Ltd – trading as Phillips Engineering – pleaded guilty to two charges of allowing unlicensed dogging work and was fined in the Fremantle Magistrates Court this week.
